ELDAC Allied Health Toolkit
ELDAC has developed an evidence-based toolkit for aged care organisations to equip them with tools, resources and guidance on palliative care practice for the support of older people nearing the end of life. The toolkit is divided into three sections: The Allied Health Team; Practising in Aged Care; and Guidance and Research.
Gwandalan National Palliative Care Program
The Gwandalan Program aims to support palliative care for Aboriginal and Torres Strait Islander communities. Resources include e-learning modules which are all approved for CPD points, face-to-face workshops, and webinars that expand on what is covered by the modules.
Meaningful Engagement Mentors Program
This program by Dementia Australia assists organisations to upskill selected staff in planning and delivering engagement activities that improve the quality of life of people living with dementia. Dementia Australia Facilitators guide staff in implementing the knowledge to projects within their organisation. Participation can be flexible, and staff are able to mentor others within their workplace to implement the same knowledge and skills.
Factsheet: Australian National Aged Care Classification (AN-ACC) Assessment Pathways
This factsheet informs permanent residential aged care providers about three AN-ACC assessment pathways for assessing and classifying an aged care resident. These pathways are dependent on whether they are a new resident entering palliative care, a current resident nearing end of life due to an event, and a current resident experiencing natural decline over time. There is also information about requirements and timeframes for assessment.
LiveUp National Ageing Platform
This free platform supports people in making the most of their lives through their ageing journey. It includes tools and information, including a quiz that can direct people to local services, and other resources.
Department of Health and Aged Care Factsheets – Free Interpreting Services
The Department of Health and Aged Care are encouraging eligible services and agencies to access Translating and Interpreting Services (TIS) due to funding being made available for this service. There are factsheets available in several languages with information on certain mental health and allied health services.
